Maternal and Child Health Handbook (Boshi Kenko Techo) will be issued to protect and control the health of the mother and the child. Condition of pregnancy, delivery, and child-rearing will be recorded in this handbook.

Procedure
If you discover that you are pregnant, please submit the Report of Pregnancy (Ninshin Todokedesho) to the following place where the report is issued as soon as possible. Maternal and Child Health Handbook will be handed to the individual who submits the report. Personal seal is not required.

The Report of Pregnancy (Ninshin Todokedesho) will be given you if you have a consultation at a medical institution in Sanda City. If you have a consultation outside of Sanda, please come to one of the following centers and fill in the report.
